best outcomes<\/li>\r\n \t<li>Initiates interventions to stabilize clients in emergent, urgent &amp; life threatening situations<\/li>\r\n \t<li>Supports, educations, counsels &amp; coaches clients regarding diagnoses, prognoses &amp; self management including responses to diseases, disorders, conditions, injuries, risk factors &amp; lifestyle changes<\/li>\r\n \t<li>Promotes self efficacy in health system navigation<\/li>\r\n \t<li>Coordinates &amp; facilitates care with other HCPs, agencies &amp; community resources<\/li>\r\n \t<li>Performs procedures for the clinical management and\/or prevention of disease, injuries, disorders or conditions<\/li>\r\n \t<li>Prescribes pharmacotherapy based on health history, disease, disorder, condition &amp; stage of life while considering individual circumstances<\/li>\r\n \t<li>Applies knowledge of pharmacotherapy &amp; evidence informed practice in prescribing, monitoring &amp; dispensing drugs (legislation &amp; organizational policy permitting for drug dispensing)<\/li>\r\n \t<li>Counsels on medication therapy, benefits, potential side effects, interactions, importance of compliance &amp; follow up<\/li>\r\n \t<li>Awareness of marketing strategies used to promote health products, medical devices, medications, alternative therapies &amp; health programs<\/li>\r\n \t<li>Intervenes when potential\/actual substance use and\/or misuse of drugs is identified (including complementary &amp; alternative therapies)<\/li>\r\n \t<li>Prescribes and\/or dispenses drugs (in accordance with provincial\/territorial and\/or federal standards &amp; legislative requirements)<\/li>\r\n \t<li>Uses an evidence-informed approach in selection\/consideration of complementary\/alternative therapies considering benefits &amp; risks<\/li>\r\n \t<li>Collaborates with clients in monitoring therapeutic response &amp; dosing adjustment as needed<\/li>\r\n \t<li>Monitors , evaluates &amp; revises plan of care &amp; therapeutic intervention based on evidence-informed practice, client goals, preferences, health status &amp;
