127 5.3 Communities & Working Groups

Working with community partners and interdisciplinary team allows for collaborative input and strategies to address gaps and barriers. ED diversion strategies include:

  • Supporting enhanced clinical knowledge & assessment to identify changes early which leads to early recognition and treatment
  • Data collection to identify gaps & inform new interventions and policy changes to support ED diversion strategies
  • Opportunity to work with community partners to visit Labs/DI frequency in the home
  • Continued analysis important to evaluate success of interventions and highlight new initiatives for continued improvement
  • Developing partnerships within the community and acute care sector
  • Palliative care
  • Specialist care via community partners
  • Engaging with community partners to facilitate direct admit vs. ED transfer
