1.7.2 Collaboration, Consultation and Referral

Collaboration, Consultation & Referral:

  • Consults with and/or refers to other health care providers (HCP) at any point in the care continuum when condition not within the NP scope of practice or NP’s competence
  • Acts as a consultant to and/or refers and accepts referrals from other HCPs, community agencies & allied health care professionals
  • Advocates around therapeutic intervention, access to care and policy decisions that affect health & quality of life
  • Collaborates with health care team members & promotes interprofessional client-centred care (individual, organizational  & system level)
  • Collaborates to promote and guide quality improvement initiatives (individual, organizational & system level)
  • Utilizes and applies advanced knowledge & skills in communication, negotiation, coalition building, change management & conflict-resolution

Research:

  • Engages in evidence-informed practice through critically appraising & applying research, best practice guidelines & theory when providing care
  • Develops, utilizes and evaluates processes in the practice setting to ensure coordinated client health services that demonstrate client outcomes & contribute to knowledge development
  • Identifies & implements research based innovation for improving client care (individual, organizational & system level)
  • Identifies, collects data & evaluates outcomes of NP practice (client and health system level)
  • Collaborates with health care team/community to identify research opportunities and to conduct/support research
  • Acts as a change agent through knowledge translation and dissemination of knowledge & develops best practice guidelines & policies[1]
