Examples of outcomes of NP leadership in LTC homes:

  • Supported establishing links between fragmented systems of care
  • Developing policies, strategies and algorithms to establish links between LTC and acute care
  • Supported emergency department avoidance programs
  • Building partnerships with other care providers and programs (e.g. laboratory, diagnostic imaging)
  • Increasing access with hospitals and specialists
  • Support staff and families

NP leadership competencies in LTC homes include:

  • Assumes complex and advanced leadership roles to initiate and guide change
  • Provides leadership to foster collaboration with multiple stakeholders to improve health care
  • Demonstrates leadership using critical and reflective thinking
  • Advocates for improved access, quality, and cost-effective health care
  • Advances practice through the development and implementation of innovations incorporating principles of change
  • Communicates practice knowledge effectively
  • Participates in professional organizations and activities that influence advanced practice nursing and/or health outcomes of a population focus
