142 6.1 Implementing Collaborative Practice Models

Models of collaboration need to be flexible to meet the needs of the various NP/MD practices, including those in LTC homes. A common structure for identifying the roles and functions of the practice partners will facilitate the education of the partners and lead to success in the implementation of practice collaboration. Based on literature reviews and input from family doctors and nurse practitioners, seven key elements form a collaboration framework:

  • Responsibility / accountability
  • Coordination
  • Communication
  • Cooperation
  • Assertiveness
  • Autonomy
  • Mutual trust and respect
