Communication in LTC:

  • SBAR is a framework often used in LTC
  • Shift change reports
  • Transfer in care communication
  • Serious illness conversations
  • End of life planning

Effective communication:

  • Prevents medical errors and saves time in the follow up for medical errors
  • Prevents duplication in service
  • Builds mutual trust with fellow colleagues and residents
  • Prevents unnecessary conflicts
  • Increases job satisfaction

Communication barriers:

  • Lack of time
  • Rotating staff
  • Staff turnover
  • Lack of established communication methods to be used by all staff
