Communication in LTC:
- SBAR is a framework often used in LTC
 - Shift change reports
 - Transfer in care communication
 - Serious illness conversations
 - End of life planning
 
Effective communication:
- Prevents medical errors and saves time in the follow up for medical errors
 - Prevents duplication in service
 - Builds mutual trust with fellow colleagues and residents
 - Prevents unnecessary conflicts
 - Increases job satisfaction
 
Communication barriers:
- Lack of time
 - Rotating staff
 - Staff turnover
 - Lack of established communication methods to be used by all staff
