182 7.6.1 Home and Community Care Partnership
National Institute on Ageing defines Home Care and Community-Support Services as:
- Care provided in home-based settings rather than in a hospital or a LTC home, with a goal to support individuals to remain independent in the community
 - Two types: home care services and home support services.
 - Coordinate with LTC services for individuals whom require 24 hours nursing services, personal care and therapeutic care: convalescent care, respite care, long term care admission
 - In most cases is controlled by the provincial and territorial governments
 - Not publicly insured through the Canada Health Act
 - In some provinces, HCC will support supplemental skilled nursing services into LTC[1]
 
A Federal Safe Long-Term Care Act is under development by the Minister of Health and the Minister of Seniors to[2]:
- Focus on results and accountability to people in Canada residing in LTC
 - Create shared responsibility of all levels of governments with respect to the well-being of older adults and persons with disabilities
 
Provincial & territorial health care governs:
- Most LTC legislation: management, organization and delivery
 - Home and community care services
 - Have most of the responsibility for delivering health and other social services (except certain groups)
 
Federal/ Health Canada is responsible for home care services to:
- First Nations on-reserve and Inuit in designated communities
 - Members of the armed forces and the Royal Canadian Mounted Police (RCMP)
 - Federal inmates
 - Eligible veterans
 - Supports research and policy analysis on home and community care across Canada
