163 Module 7: Optimizing Health Systems in LTC

In this module, you will review the Advanced Practice Nursing (APN) Framework, optimizing health systems and its relation and application to NP practice in the LTC setting. This module is designed to support in gaining knowledge related to the LTC setting, legislation, learn practices specific to LTC and appreciate and understand opportunities to further optimize the NP role in the LTC setting while supporting evidence based resident centred care. You will also explore opportunities to support, guide, promote and enhance LTC practice through health systems partnerships and integrated care.

This module is offered through asynchronous learning format including self-directed and reflective learning, videos, resident examples and pre and post-test evaluation to support your learning needs.

Learning Objectives

This module will enable nurse practitioners, primary care practitioners and students to:

  1. Review LTC legislation & regulation in LTC and its relation to NP practice.

     

  2. Apply equity, diversity, inclusivity & truth and reconciliation initiatives in LTC practice.

     

  3. Discuss practices unique to LTC practice including: Mandatory training; Critical incidents & mandatory reporting; Integrated documentation; InterRAI assessment; Virtual care; and Referral pathways.

     

  4. Explore and review health system partnerships and application of integrated care in the LTC setting including: Community care partnerships; Geriatric program partnerships; Acute care partnerships; and Resident/SDM, family & caregiver partnerships.
